General Policies

• Co-Pays and deductibles are due at the time of your appointment.
• We ask you to arrive 10 to 15 minutes prior to your appointment so that your records may be updated.
• Lab results are returned directly to our office. Once we have analyzed the test results, we will contact you.
• Prescription/Refill requests for pain medication, antibiotics, and any controlled substance REQUIRE an office visit.
• All medications taken on chronic basis require regular follow-up visits.
• Unfortunetly, we do not take any Workman’s Compensation or Motor Vehicle Accident cases.
• Medication refills require a 48 hour notice.
